It is Clover's policy that an investigation of all work related accidents, injuries and illnesses are to be conducted in a professional manner in order to identify probable causes and develop specific management actions for the prevention of future accidents.

All employees must contact Clover Management IMMEDIATELY in the event of an accident or medical emergency at 281-797-9932. This number is monitored 24 hours a day/7 days a week and is for Emergency Use Only. Employees must then complete the appropriate Accident Report Forms (see links below).

Due to the nature of the Clover businesses model, Clover Management will work closely with the client's investigation team as deemed necessary. Integration of the Clover Accident Reporting and Investigation programs with the client's systems at the site which you are working is vital. To ensure effective communication, you are not only required to complete and submit the appropriate Clover forms but also include a copy of the client's report.

Report a Serious Accident:

• Fatality
• Hospitalization of three or more Clover Employees
• Permanent total, permanent partial or temporary total disability
• Property damage in excess of $50,000

Report an Other-than-Serious Accident:

• Injuries and illness that did not result in hospitalization or fatality
• Injuries that require minor treatment (First Aid)
• Property damage less than $50,000
